We realise that having a long-term condition for which you may have to take several tablets every day is bad enough, without being reminded about it by invitations to see members of the Practice team. Nevertheless we make no apology for making the invitations. Health needs change with time, medication can have adverse effects as well as beneficial ones and knowledge about how best to care for long-term conditions improves all the time.
The conditions that we are particularly keen to keep under regular review are those where good management can result in better health. These include:
In order to look after your condition well we shall invite you from time to time to a review with the doctor or the nurse. This is usually once every 6 months but may only be once a year for patients with epilepsy, underactive thyroid, raised cholesterol or some patients with asthma. Your review will include a discussion about your condition and medication, examination if appropriate, e.g. blood pressure, and blood tests where they are indicated.
The reviews can be with either of the doctors or either nurse but we do have special interests that will influence who invites you. In particular Sister Maureen Bylo runs a Thursday morning clinic for patients with cardiovascular disease, asthma and COPD, for which you may receive an invitation. Sister Wendy Lees sees patients with diabetes on a Tuesday afternoon.
