New Patient Registration

registrationIf you wish to register with the Practice please come and speak to one of our receptionists – or ask someone else to do this on your behalf.  You will be given a form to fill out.  This requests all the information that we need in order to register you as a patient, including the details of your last doctor so that we can obtain your medical records.  Please fill out the form and return it to the receptionist.  You will also be asked to complete a new patient questionnaire and return it to the receptionist.  This is because it can take several weeks for us to receive your medical records.  If you wish to complete the registration form and new patient questionnaire now these can be printed out using the links below.

You will be registered with the Practice and may see any one of the doctors for consultations.  If you wish you may state a preference for seeing any one of the doctors and this will be recorded on your computer record.  When you make an appointment please make it clear if you wish to see a particular doctor.  At certain times this may mean that you are not able to get to see your preferred choice of doctor within two working days, but you will always be able to see a doctor within this time.
