Contact us online without an account

You don’t need an online account to contact us online.  Just follow this link to contact us about:

  • fit (sick) notes
  • recent test results
  • repeat prescriptions
  • anything else admin related

You can also ask for advice about a new symptom or ongoing condition or treatment as long as you don’t need an urgent response.

 

Get an online account

An online account allows you to securely view parts of your medical record such as allergies, test results, and immunisation history as well as book appointments, and request repeat prescriptions.  For further information about online accounts please read our Information Leaflet.  In summary, there are two ways to set up an online account:

  1. The NHS App (recommended method for patients at Topcliffe Surgery)
  2. Topcliffe Surgery online account via Patient Access or Evergreen Life.  Bring the completed registration form into the surgery. All patients are asked to provide two forms of identification; one photo-ID and another item such as a utility bill or bank statement. Please remember to bring your identification documents with you at the same time as your registration form.  We will then email you further instructions.
